There is no application form and no test to pass. But a little context helps us give you a useful answer from the start. Tell us whatever you already know.

The shoe What model, shape or general idea are you interested in?
¿Made to Measure? If standard sizes usually work for you — or definitely do not.
Your country So we can factor in fitting and shipping from the start.
Budget A rough range helps us recommend the right materials, not simply the most expensive ones.
Your feet Anything unusual or important that we should know.
References A photograph, a drawing, an old shoe or even a rough idea can be useful.

One last thing

We are a small workshop and can only take on a limited number of commissions. We are not trying to create urgency; it is simply a matter of capacity and giving each project the time it needs.
